Community Support | Case Manager

Entering its 30th year of service, Starfire is a nonprofit organization that empowers leaders to build community and inclusion alongside people with developmental disabilities. We do this by increasing social connectivity through meaningful relationships and by working to remove barriers to opportunities in the community where people can be known for their gifts, not their disabilities.

Starfire is seeking a full-time Community Support/Case Manager, to support people with disabilities and their families to identify and leverage their assets to create connections within their community based on common interests. Through this work, you will help the individual and their family foster engagement of potential relationships by creating interesting projects based on passions and interests, and over time, a network of social connections within their community.

Each week, you will meet one-on-one with individuals who have developmental disabilities with the stated goals of exploring interests to match and secure opportunities to create valued social roles and connections, collaborating with community members who may or may not have a developmental disability, and nurturing and maintaining newly created “circles” of relationships.

Though the details of the position will change with each individual person that you will support, the following provides an overview of duties and responsibilities:

Essential Duties & Responsibilities

  • Possess ability to connect, build trust and common ground with persons with disabilities, family and community members, Starfire staff, and others, regardless of race, ethnicity, religion, economic, and disparate interests, while displaying curiosity, respect, empathy, and joy in all facets of community building.

  • Possess an adaptive, design-thinking mind-set—the ability to think creatively about how to solve problems, overcome obstacles, motivate people, and ultimately find the successful path to building a “circle” or social network along with people with varying levels of ability.

  • Meet one-on-one with individuals with developmental disabilities weekly to path each through the Starfire stages of exploration, collaboration, and maintenance of their “circle” or social network.

  • Investigate opportunities for valued social roles, organize and facilitate meetings and conversations with potential connections within the community and each individual supported with a developmental disability.

  • Assist relationship development, communication, and reciprocity (ie: making invitations, sending thank you cards, birthday recognition, offering to buy coffee, making invitations) with the “support network” of each individual supported with a developmental disability.

  • Establish a weekly communication cadence with the family of each individual with a developmental disability and Starfire staff to share reflections, progress against stated goals, opportunities for additional capacity building.

  • Share success stories, photos etc, for social media and grant reports.

  • Complete documentation of services and metrics as required.

  • Exercise independent judgement and initiative while possessing a strong sense of self-motivation and discipline to regularly set and achieve goals.

  • Maintain budget allowance and documentation for program.

  • Attend weekly meetings with supervisor, monthly staff meetings, workshops and trainings as required.

  • All other duties as assigned

Minimum Qualifications

  • BCII and FBI background check

  • BMV Driving Record

  • Other related background checks

  • CPR/First Aid certified (training provided)

  • Reliable transportation and use of personal vehicle (mileage reimbursed)]

The Community Builder role is a full-time position, and reports to the Community Builder Network Coordinator.
